Complicated idiopathic portal and mesenteric venous thrombosis: A case report

Abstract Introduction : Acute mesenteric ischemia is a life-threatening condition that can lead to intestinal ischemia, bowel obstruction and peritonitis. It predominantly caused by arterial occlusion (acute thromboembolism or thrombosis); however, it rarely be secondary portal vein thrombosis. Case presentation We present the case of 61-year-old man admitted emergency service for intense abdominal pain lack movement. A computed tomography angiography (CTA) was performed, revealing Anticoagulant treatment established. To appraise extension necrotic tissue, surgical team performed exploratory laparotomy, followed segmental enterectomy. The following day, patient complained acute in left lower limb CTA indicating ischemic phenomenon. thrombectomy using Fogarty catheter. enteral region extended, developed an enterocutaneous fistula required reintervention (enterectomy right hemicolectomy). Conclusion Conservative with anticoagulants first line treatment, only complications. Even though venous thrombosis exceptional cause infarction, complications such as necrosis, peritonitis septic shock.
